VO₂ max is the maximum amount of oxygen your body can take in and use during all-out exercise, measured in millilitres of oxygen per kilogram of bodyweight per minute (mL/kg/min). It reflects how well your lungs, heart, blood and muscles work together to deliver and burn oxygen, which is why it is regarded as the single best laboratory measure of aerobic — or cardiorespiratory — fitness.
Its importance goes well beyond sport. A higher VO₂ max is one of the strongest independent predictors of lower all-cause mortality and better long-term health, on a par with or ahead of traditional risk factors. In other words, it is as much a vital sign as it is a performance metric — which makes having a reasonable estimate genuinely useful even if you never race.
Measuring VO₂ max directly requires a maximal exercise test wearing a gas-analysis mask in a lab — accurate, but costly and inaccessible to most people. Clinarix instead uses the Uth–Sørensen method, which estimates VO₂ max from the ratio of your maximum heart rate to your resting heart rate: VO₂ max ≈ 15.3 × (HRmax ÷ HRrest). We derive your maximum heart rate from your age using the Tanaka equation (208 − 0.7 × age), which predicts maximum heart rate more accurately across adult ages than the older "220 − age" rule.
The logic is elegant: a fitter heart has a larger stroke volume, pumping more blood with each beat, so it does not need to beat as often at rest. That is why a low resting heart rate signals high aerobic fitness, and why the ratio of maximum to resting heart rate tracks VO₂ max so well. The method was validated against directly measured VO₂ max in trained and untrained adults and gives a sound estimate for healthy people without any equipment beyond a way to count your pulse.
The quality of the estimate depends almost entirely on the quality of your resting-heart-rate input, so measure it properly: take your pulse first thing in the morning, before getting up and before caffeine, lying still for a full minute — or read it from a wearable's overnight resting figure. A reading taken after coffee, stress or movement will be elevated and will understate your fitness.
Use the result as a baseline to improve rather than a fixed verdict. The most effective way to raise VO₂ max is a mix of higher-intensity interval work and a base of easier aerobic training; as your fitness climbs, your resting heart rate falls and your estimate rises with it. This is an estimate for healthy individuals, not a diagnostic test. Age-predicted maximum heart rate carries meaningful individual variation, and the estimate is unreliable for people on heart-rate-altering medication such as beta-blockers, those with atrial fibrillation or other arrhythmias, and anyone with a pacemaker — in these cases the heart-rate inputs no longer mean what the formula assumes. Before starting vigorous or maximal exercise, people with known heart disease, uncontrolled blood pressure, or symptoms such as chest pain or unexplained breathlessness should seek medical clearance. For a true clinical assessment, a supervised cardiopulmonary exercise test is the appropriate tool.
